Primary Responsibilities of an LMSW Provide case management and
social work services aligned with the needs and goals of clients,
focusing on holistic and client-centered approaches. Collaborate
with multidisciplinary teams, including healthcare providers, to
deliver integrated care solutions. Conduct assessments to identify
the social, emotional, and environmental needs of clients, adapting
plans to meet their evolving circumstances. Develop and implement
intervention strategies and support programs tailored to individual
and community needs. Maintain detailed and accurate documentation
of all client interactions and interventions, ensuring compliance
with regulatory standards. Qualifications for a Successful LMSW
Master's degree in Social Work from an accredited institution.
Valid LMSW license in the state. QMHP or CMHP from state of MI
Proven experience in providing social work services in healthcare,
community, or social service settings. Excellent communication and
interpersonal skills, with the ability to work effectively in a
team-oriented environment. Strong organizational skills and ability
to manage multiple cases effectively. Work Schedule: ? Monday
through Thursday 8:30 am to 7 pm and Friday 8:30 am to 5 pm.
Evening hours will be required, and determined based on program
