Physical Therapy Assistant (PTA) Job Description At h/care, we are
excited to announce a full-time job opening for a dedicated and
compassionate Home Health Physical Therapy Assistant (PTA). This
position is ideal for individuals passionate about providing direct
patient care in a home health setting, helping patients regain
movement and strength after injuries or illnesses. As part of our
committed home health team, you will work under the supervision of
registered physical therapists to ensure that each patient's care
plan is executed effectively. This is not a remote position, and it
requires working directly with patients in their homes, creating a
critical impact during their recovery process. As a Home Health PTA
at h/care, you will be at the forefront of our mission to deliver
outstanding physical therapy services outside traditional
healthcare facilities. This role is suited for those who are
adaptable, enjoy working in varied environments, and are committed
to the healing and improved well-being of patients in their
personal environments. Your contributions will directly help
individuals achieve better health outcomes and enhanced quality of
life. Duties and Responsibilities Assist physical therapists in the
implementation of treatment plans prescribed by the patient’s
physician. Provide direct therapy interventions based on the
patient’s plan of care to improve mobility and decrease pain.
Monitor and document patient’s progress, and report any changes or
recommendations to the supervising therapist. Teach and demonstrate
exercises meant for pain relief, strengthening, and improved
mobility. Engage with patients, caregivers, and family members to
educate them on the patient’s therapeutic regimen and ensure safety
practices are followed at home. Operate and instruct patients on
how to use therapeutic devices and equipment. Travel to patient
homes within the assigned area to provide hands-on therapy
treatments and patient care. Maintain patient confidence and
protects operations by keeping information confidential. Adhere to
professional and ethical standards as set by the governing body.
Participate in scheduled training sessions and maintain relevant
certifications and licensure. Requirements Associate’s degree from
an accredited PTA program. Current and valid state license as a
Physical Therapist Assistant. Minimum of two years of work
experience as a Physical Therapist Assistant, preferably in home
health or related settings. Strong understanding of physical
therapy modalities, patient management, and treatment protocols.
Exceptional communication skills, both verbal and written, for
effective interaction with patients, families, and healthcare
staff. Ability to work independently in a home setting and manage a
geographically assigned caseload. Demonstrated ability to make
sound judgments and implement care plans effectively. Current CPR
certification. Valid driver’s license and reliable transportation,
with willingness to travel within the designated service area.
Physical ability to handle frequent lifting and/or transferring of
patients as needed.
